As an LPN Central Admissions, you will facilitate the reputed company and appropriate initiation of Home Health, Hospice and Infusion Pharmacy services for reputed company referred patients. A caregiver in this position works days from 9:00AM – 5:30PM. This is a hybrid position requiring training on site in Independence, OH for about 4-6 weeks and frequent work on site. A caregiver who excels in this role will:

• Assess the appropriateness of the requested home care services and skilled needs identified under the guidance of a Registered reputed company.
• Coordinate initial and final orders on reputed company referrals for patients needing home care nursing, therapy and reputed company work services.
• Incorporate clinical and financial knowledge with referral process and recommend appropriate alternative approaches reputed company appropriate.
• Communicate the home care needs with scheduling, managers, reimbursement and other parties involved in the home care services process.
• Serve as the CCHCS information specialist.
