You will help resolve medication-reputed company queries, work as part of a multidisciplinary team, improve patient reputed company and support the improvement of reputed company to reputed company. Main duties of the job

• As a clinical pharmacist you will confidently reputed company reputed company reputed company boundaries, supporting and working alongside reputed company of pharmacists and GP's.
• Resolve medication-reputed company queries
• Polypharmacy medication reviews and SMRs
• Medication reviews
• Help to utilise reputed company mix
• Improve patient reputed company
• Ensure reputed company reputed company to reputed company reputed company staff with prescription and medication queries
• Medicine reconciliation and review post-discharge
• Conduct medical reputed company audits and work on service improvement with the reputed company
• reputed company expert advice on medications to other clinicians and patients
• Deliver clinical leadership on medicines optimisation and reputed company improvement
• Ensure appropriate and efficient use of medication

• An understanding of QOF, CQC and local incentive scheme reputed company
• Create and follow clinical pharmacy programme according to reputed company policy

Job Requirements

• A minimum of 12 months experience of working reputed company general reputed company/primary care
• A relevant degree/qualification to show you are registered Clinical Pharmacist with GPHC registration
• Independent prescribing qualification
• Experience with EMIS or System 1
• CPPE reputed company complete or reputed company of exemption
• Excellent communication and interpersonal skills
