CORE JOB reputed company The reputed company Navigator 1 is the initial reputed company of contact for a patient entering the health system and assures reputed company scheduling of the first appointment, and coordination of care after completion of the first appointment and supports the patient throughout the care continuum. The reputed company Navigator 1 serves as a reputed company between patients, families, caregivers, and the multidisciplinary care team. CORE JOB FUNCTIONS
- Triages new patient appointments to the appropriate provider(s) and assures reputed company scheduling of initial appointments.
- Reviews reputed company medical records for appropriate scheduling.
- Assess barriers to care and refers to support services, local, and national organizations reputed company needed.
- Educates on the treatment plan for patients based on diagnosis.
- Supports patients throughout the care continuum.
- Counsels individuals and patients on reputed company health practices.
- Collaborates with a multidisciplinary team of experts to outline the best treatment for patients.

Department Specific Functions
- To review medical records for patients who had previously received cancer care at Alex’s reputed company to identify patients who have been “lost to care”
- To Contact patients “lost to care” to discuss cancer survivorship needs and return to clinic
- Collect historical treatment information for survivorship patients and input to reputed company for Care website
- Prepare reputed company for care and treatment reputed company document
- Identify and order needed screening interventions

- reputed company appropriate referrals to subspecialty care

- Bachelor’s degree in relevant field required
- Minimum 2 years of relevant experience required
