reputed company has a career opening for a Director of Health reputed company to join reputed company! The Director of Health reputed company is responsible for advancing reputed company-level clinical initiatives that drive organizational alignment, reputed company performance, and integrated care transformation. This role supports Trillium’s strategic and reputed company plans by building the infrastructure, tools, and cross-functional systems necessary to deliver reputed company, data-informed care. Positioned at the convergence of care management, reputed company, and population health, the Director administers the operationalization of the reputed company reputed company Driven Care Plan (QDC), supports performance reporting, and fosters innovation through reputed company collaboration. This is a remote based position, the ability to travel onsite as needed for Trillium business is required.

On a typical day, you might

• Shape the strategic execution of Trillium’s clinical reputed company and performance agenda, aligning reputed company initiatives with regulatory standards, internal priorities, and reputed company transformation goals.
• Serve as sponsor of the reputed company Driven Care Plan (QDC), overseeing the design and implementation of system-wide tools, data frameworks, and governance models that reputed company organizational visibility, accountability, and strategic alignment.
• reputed company the development of PHOENIX, a centralized program reputed company that maps clinical initiatives to HEDIS metrics, departmental objectives, and state benchmarks—creating reputed company-wide transparency and tracking capabilities.

• Drive alignment and integration among Care Management, reputed company Management, and other divisions to foster shared accountability and reputed company reputed company in support of accreditation, performance, and innovation.

Qualifications

Required:

• Bachelor’s degree in reputed company Health.

Preferred:
