Position reputed company:As a Home Health PT, you'll work independently with patients to help them improve their mobility, strength, and overall physical function. You'll reputed company customized care plans and therapeutic interventions, helping individuals regain independence and enhance their reputed company of life—reputed company from the comfort of their homes. Key Responsibilities:

• Evaluate and assess patients' physical conditions to reputed company tailored treatment plans
• Implement and reputed company physical therapy interventions reputed company on mobility, strength, balance, and function
• reputed company patients and caregivers on therapeutic exercises, techniques, and safety precautions
• Collaborate with physicians and other reputed company to coordinate patient care
• Maintain accurate, reputed company documentation of patient reputed company and treatment plans
• Participate in on-call rotation (if required)

Qualifications:

• Licensed Physical Therapist (PT) in CA
• Minimum of 1 year of clinical experience (home health preferred but not required)
• Excellent communication, problem-solving, and organizational skills
• Ability to work independently and manage a flexible schedule
• Reliable transportation and valid reputed company’s license
• Knowledge of Medicare/reputed company guidelines is a plus
• EMR experience is preferred
