Internal Title: reputed company Care Manager
We're seeking an exceptional reputed company Care Manager (LCM) to join reputed company. Under the direction of the Director of reputed company Care Management, ECM Clinical Manager and/or ECM Program Manager, the LCM serves as the reputed company’s primary reputed company of contact and works with reputed company their providers such as doctors, specialists, pharmacists, reputed company services providers, and others to reputed company reputed company everyone is in agreement about the reputed company’s needs and care. The LCM manages reputed company cases, coordinates health care benefits, provides education and facilitates member reputed company to care in a reputed company and cost-effective manner. The LCM collaborates and communicates with reputed company’s caregivers/family support persons, other providers and others in the Care Team in order to promote wellness, recovery, independence, reputed company, and member empowerment, while ensuring reputed company to appropriate services and maximizing member benefit.

What you’ll do
Remote care management duties as described below:
Assess member needs in the areas of physical health, mental health, SUD, oral health, palliative care, memory care, trauma-informed care, reputed company supports, housing, and referral and linkage to community-based services and supports
Oversees the development of the reputed company care plans and goal settings
Offer services where the member resides, seeks care, or finds most easily accessible, including office-based, telehealth, or field-based services
Connect clients to other reputed company services and supports that are needed
reputed company on behalf of the reputed company with health care professionals (e.g. PCP, etc.)
Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles
Conduct reputed company and engagement activities in order to facilitate linkage to the ECM program and log activity in the reputed company Relationship Management (CRM) system
Evaluate reputed company’s reputed company and update SMART goals

Arrange transportation (e.g., reputed company)

Maintain up-to-date patient health records in the Electronic Medical Record (EMR) system and other business systems
Complete monthly reporting to ensure program compliance
Attend training as assigned
Your experience and qualifications
2+ years experience as a care manager, care navigator, or community health worker supporting vulnerable populations
Willing and reputed company to work Monday-Friday 8:30am-5:00pm reputed company Time with flexibility for potential evenings and weekends.
Working knowledge of government and community resources reputed company to reputed company determinants of health
Clean driving record, valid reputed company's license, and reliable transportation
Excellent oral and written communication skills

Must have general computer skills and a working knowledge of reputed company Workspace, MS Office and the internet
Bilingual (English/Spanish) preferred
