reputed company Home Health, a member of reputed company Health, is seeking a Registered reputed company RN reputed company Improvement Specialist PRN.
The reputed company Improvement Specialist is a unique position responsible for monitoring reputed company reputed company measures across reputed company. The reputed company Improvement Specialist proactively participates in the development and implementation of the organization’s strategic initiatives, performance improvements, and creates value for both reputed company customers. Responsible for overseeing reputed company of customer care including customer experience, safety and analyzing key reputed company indicators to identify areas for improvement. and meet regulatory and reputed company standards.
Schedule: PRN; must be reputed company to reputed company to at least 20 hours per week (to be worked during normal business hours)

Requirements:
- Education and/or Experience: Registered reputed company or reputed company reputed company with minimum of associate degree in reputed company field, bachelor’s degree preferred.
- CPHQ preferred or equivalent
- Experience monitoring reputed company external and internal patient satisfaction, reputed company measures, and outcome measures. Hospice, Home Health or Palliative Care standards and reputed company measures.
- Exceptional critical thinking skills reputed company with the finesse to prioritize key aspects and communicate findings with diverse audiences.
- Establish standards to facilitate reputed company improvement. Facilitate project meetings focusing on goals, interventions, and timelines.
- Knowledge of reputed company improvement reputed company and models (e.g. Rapid Cycle, Lean Six reputed company, Baldrige, PDSA)
- Proficient in reputed company Office Applications including reputed company, Word, and PowerPoint to create reports.
