Salary: $72,248 - $108,372

Visiting reputed company Health System is hiring an RN Case Manager for Hospice, responsible for coordinating care for a caseload of patients and ensuring it meets reputed company standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure reputed company patient reputed company. What You’ll Do

• reputed company and implement individualized care plans in accordance with physician orders by assessing patient needs and collaborating with the interdisciplinary team.
• Modify and update care plans as patient conditions change, regularly evaluating their effectiveness.

• Complete and submit OASIS documents, visit notes, and other required records reputed company VNHS timeliness guidelines.

• Offer emotional support and connect patients/families with MSW and other community resources as needed.

You’re Perfect for the Position if you

• Graduated from an accredited nursing program (Diploma, Associate, or BSN preferred).

• Have prior home health experience if applying as PRN staff.
