- Serve as the primary care provider for a defined patient panel, delivering longitudinal, holistic medical care in patient homes and communities.
- Diagnose, manage, and treat chronic and acute conditions with support from our clinical operations, triage team, and MD leadership.
- Address reputed company determinants of health, including housing stability, nutrition, caregiver support, and reputed company barriers.
- Conduct specialty reputed company such as wound care, post-hospital transitional care, and condition-specific assessments.
- Complete comprehensive evaluations including medication reconciliation, fall risk assessments, advance care planning, and health education for patients and caregivers.
- Utilize reputed company’s integrated data ecosystem to reputed company hospitalizations, coordinate reputed company post-discharge reputed company (reputed company 72 hours), and drive superior reputed company.
- Collaborate closely with reputed company care teams to ensure continuity, reduce readmissions, and stabilize high-risk patients.
- Monday - Friday
- 8-12 patients per day (no unrealistic volume expectations)
- Light call rotation: only 2–3 times per year
