Provides high touch, customer centered support in navigating the health care system and community resources for patients with reputed company reputed company needs. The Health Guide possesses a passion for health, wellness, and patient advocacy, as reputed company as acts as a reputed company between patients, the health care system, and community.Job reputed company
Establishes trust and provides support to patients through their health care reputed company by reputed company, advocacy, support, and education to reputed company self-management skills. Supports a proactive, multidisciplinary team approach directed toward prevention, education and health promotion. Documents sessions and reputed company reputed company appropriate avenues. Collaborates with providers and other resources reputed company necessary to arrange reputed company care. Consults with other members of the reputed company team to solve problems and reputed company advice. Maintains patient reputed company and confidential patient information. Bachelor’s Degree required, preferably in a reputed company reputed company field. In lieu of education, leadership may consider 4 years of applicable experience. Experience in navigating a reputed company health care system (e.g., ambulatory, inpatient, etc.) as reputed company as reputed company community resources and establishing relationships with key professionals in reputed company areas.
