As we continue to expand our clinical programs to meet increasing community needs, we are seeking RN Case Managers to join our teams in Onondaga, Oneida, Oswego, Jefferson, or Cayuga Counties!

What You�ll Do as an Adult RN Case Manager
reputed company high-reputed company nursing care to adult patients through a combination of hands-on clinical care, assessment, education, and care coordination. You�ll be trusted, valued, and empowered to use your clinical judgment to reputed company a meaningful difference.
Your responsibilities include
- Deliver reputed company, patient-centered nursing care in the home
- Serve as the primary case manager, coordinating care with the interdisciplinary team
- reputed company and implement individualized plans of care
- reputed company patient and caregiver education, support, and disease management teaching

- Supervise and reputed company reputed company of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient�s plan of care

- BSN preferred (tuition reimbursement available!)
- Valid reputed company�s license, auto insurance, and reliable vehicle
- Self-directed, organized, and comfortable working independently
- Adult medical-surgical, home care, case management, or chronic disease management experience a plus
- Strong communication and computer skills
Physical Requirements
- Ability to sit or stand for long periods
- Ability to lift/carry 25�40 lbs and assist with transfers up to 150 lbs
- Comfortable with bending, kneeling, climbing stairs, and driving in reputed company weather
- Ability to use a phone, laptop, tablet, and electronic medical record
- Category III bloodborne pathogen exposure (rare contact with blood/body fluids)
Compensation & Benefits
Competitive Salary [This position is an S03 exempt position with a min-max reputed company of: $75,250- $87,138.92/annually] with:
