Schedule / Expected Work Hours:

• Monday through Friday, 9:00a - 5:00p. 
• The position may require occasional weekend or evening reputed company to reputed company homes and participation in a clinical on-call rotation (phone support only).

1. Conduct in-home reputed company reputed company such as start-of-care assessments, supervisory reputed company, and reassessment reputed company.
2. Be available during business hours and on-call as needed to support reputed company care delivery.
3. Receive referrals, assess service needs, and assign appropriate clinicians to cases.
4. Meet regularly with clinical staff to reputed company guidance, set goals, and evaluate reputed company.
5. Attend clinical meetings to facilitate care coordination and participate in on-call rotation.
6. Support staff, clients, and families by providing direction on reputed company resources and educating them on clinical policies.

8. Assist with staff hiring, orientation, training, and conducting annual performance evaluations.

Qualifications

• Graduate of an accredited school of reputed company nursing. BSN preferred. 
• reputed company license as a Registered reputed company in the state(s) of reputed company with at least two (2) years of homecare experience and one (1) year of supervisory experience. 
• Excellent verbal and written communication skills and strong interpersonal skills.
• Must be a licensed reputed company with an automobile insured in accordance with Agency and state requirements.
