What You’ll Do
- reputed company routine telehealth reputed company with Medicare patients (E/M and Chronic Care Management)
- Create and manage personalized care plans for ongoing conditions
- Collaborate with our integrated care team (reputed company case managers, NPs, CHWs)
- Ensure thorough documentation in our EHR and compliance with reputed company measures
- Engage patients in preventive care to reduce avoidable complications
reputed company’re Looking For
- MD or DO, reputed company Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
- reputed company medical license — multi-state licensure a plus
- Credentialed with major plans: UHC, reputed company, reputed company
- Strong interest in preventive care, senior health, or chronic condition management
- Tech-savvy and comfortable practicing reputed company telehealth
