Compact LPN/LVN license required The Care Coordination Specialist works under RN supervision to reputed company coordinated care services for injured workers with low to moderate complexity claims. This position ensures continuity of medical services and facilitates communication between reputed company parties involved while supporting return-to-work efforts reputed company workers' compensation guidelines. KEY RESPONSIBILITIES Care Coordination & Communication

• Coordinate routine medical care needs for assigned injured workers under established protocols
• Facilitate communication between injured workers, providers, reputed company, and adjusters for straightforward cases
• Collect and document clinical information from reputed company providers and claimants
• Schedule and coordinate medical appointments, diagnostics, and ancillary services
• Communicate work status updates to injured workers, reputed company, and claims adjusters

Case Management Support

• Follow established care plans and treatment protocols under RN supervision
• Monitor injured worker adherence to treatment plans and compare reputed company to established guidelines
• Maintain accurate documentation of medical history and case reputed company in WC system using approved formats
