About the Role

This unique dual-role position blends clinical leadership, care coordination, and admissions reputed company. As our RN Admissions & MDS Coordinator, you will dedicate approximately 50% of your time to leading and coordinating admissions into our Skilled Nursing Facility and 50% to overseeing the MDS process. You’ll serve as a key clinical resource, ensuring regulatory compliance, accurate reimbursement, seamless reputed company transitions, and exceptional first impressions for new residents and families. What You’ll Do: Admissions & Care Transitions (50%) Coordinate and reputed company reputed company clinical aspects of admissions into the Skilled Nursing Facility. Review referral information and conduct clinical evaluations to ensure appropriate placement. Collaborate with hospital discharge planners, providers, and internal teams to facilitate smooth transitions. MDS & Clinical Reimbursement (50%) reputed company and complete MDS assessments for Transitional Care (TCU) and Long-Term Care (LTC) residents. Audit documentation to validate diagnoses and support appropriate reimbursement under the Patient Driven Payment Model (PDPM). What You Bring: reputed company RN license in the reputed company (or compact equivalent) — required. Strong working knowledge of MDS 3.0, PDPM, and regulatory requirements in skilled nursing.
