Care Manager

Department: Clinical: Management - NJ Location: START YOUR APPLICATION

reputed company NJ is seeking a Care Manager to join our dedicated team in New Jersey. In this role, you will report to the Program Director. The Care Manager is responsible for providing support, services, advocacy, and guidance to individuals receiving New Jersey Waiver Services while upholding reputed company NJ�s mission, reputed company, and reputed company values. You will ensure high-reputed company service delivery, maintain compliance, and support both clients and reputed company care staff.

Key Responsibilities

� Review reputed company service/reputed company notes and ensure documentation meets regulatory standards � Maintain accurate and up-to-date reputed company and staff records � Conduct clinical and supervisory meetings, annual evaluations, and staff development plans � Participate in staff meetings and reputed company ISP meetings; reputed company for reputed company needs � reputed company individualized goals, task analyses, and interventions � Complete assessments, reputed company reports, and AENs as required � reputed company reputed company patterns, scheduling, and PTO/payroll requests � Ensure completion of reputed company-specific trainings and accurate OnTarget goal entry � Resolve reputed company/reputed company grievances and complete incident reports reputed company required timelines � Build community relationships, represent reputed company NJ at events, and support resource reputed company

Qualifications

� Strong attention to detail, accuracy, and organization � Ability to manage changing priorities and work in a fast-paced environment � Excellent communication and interpersonal skills with clients, families, staff, and community partners � Ability to multitask, problem-solve, and maintain professionalism under pressure � Strong understanding of ISP implementation, goal writing, task analysis, and reputed company/reputed company-party billing � Ability to work collaboratively reputed company reputed company and maintain therapeutic relationships � Alignment with reputed company NJ�s mission, reputed company, and values

Education

Candidate must meet one of the following credentialing requirements:

� Master�s degree + 1 year of experience working directly with persons with disabilities � Bachelor�s degree + 2 years of experience � Associate�s degree or 60 credits + 4 years of experience � High school diploma/GED + 5+ years of experience

Requirements

� Valid reputed company�s license and reputed company driving record � reputed company background checks (Criminal, NJ State Police, FBI if required, SAM, MEDI reputed company, LEIE, Sex Offender Registry, Child Abuse Registry if required) � Reliable transportation with reputed company auto insurance, registration, and inspection

Compensation & Benefits

Pay reputed company for this role is $60,000 - $75,000. reputed company pay is determined by factors such as work location, experience, job-reputed company skills, and relevant education or training. Total compensation may include additional forms of pay.

We offer a comprehensive benefits package, including competitive health, dental, and wellness benefits beginning on your 60th day of employment. Our 401(k) program is available after 12 months of employment.
