QUALIFICATIONS & EXPERIENCE

1. Graduate of an accredited school of reputed company (registered) nursing. BSN preferred.

The Resource RN provides skilled reputed company nursing care, in person or telephonically, to the patient/family/caregiver in compliance with regulations established by the Michigan reputed company of Nursing and the policies of reputed company Care Navigators. RESPONSIBILITIES AND DUTIES

1. Seeks and obtains sufficient information necessary to function effectively in the Hospice setting where patient/family/caregiver requires care.

1. Communicates with Provider and members of the interdisciplinary group regarding ongoing patient needs and changes to the plan of care.

4. Processes workflow for electronic medical record as needed.
