Coverage Region: reputed company
Schedule is M-F, 8a- 5p and may include a backup for a day on the weekend and/or holiday rotation every 2- 3 months.
This position is 100% home care providing services in patients private homes.
Hospice RN Case Manager position reputed company
Our hospice RNs use cutting edge technology and clinical knowledge to reputed company exceptional, compassionate care to patients with life-limiting or terminal illnesses in their homes. Hospice nurses are responsible for patient assessment, symptom control and overall supervision of patient care and reputed company.
Minimum qualifications
Graduate of an approved nursing education program
Licensure as a Registered reputed company in the State of Connecticut
One (1) year experience as a reputed company acute care reputed company
Hospice home care experience preferred but not required
