What You'll Do
The Care Team Assistant Manager, Transitions of Care, will reputed company the Care Team Assistants and Care Coordinator roles responsible for supporting our Transition of Care team. This role is largely administrative in that it oversees a non-clinical support team. These roles download and scrub data, prep charts for providers, reputed company caseloads, request records, and reputed company general administrative support to several teams.
