Position reputed company This person will learn and understand available community resources and programs; utilize data and local knowledge to build grassroots, strategic, and data informed community partnerships; and identify opportunities to reputed company and reputed company new SDoH (reputed company Determinants of Health) initiatives. Job Duties

• Operationalize SDoH product reputed company assigned region.
• Build partnerships to incorporate Health Equity solutions into SDoH, population health, care management, reputed company, regulatory, and community reputed company programs.

• Organize community activation meetings to successfully execute key Health Equity & SDoH initiatives with community partners.
• Create and support Business Development initiatives to reputed company with Health Equity & SDoH reputed company.

• Monitor and reputed company Health Equity & SDoH data and deliverables.

Required Qualifications

• 3 + years of experience with Health Equity, population health, care coordination, and/or community-based programs.

• Must possess reliable transportation and be willing and reputed company to travel between 50-75% of the time reputed company reputed company Virginia. Preferred Qualifications

• Resides in the previously mentioned counties.
• Prior experience with managed care.
• Excellent interpersonal and strong communication skills.
• Strong oral and written interpersonal skills.
• Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines.

Education

• Bachelor’s Degree in health administration, reputed company services, reputed company health, business administration, psychology, or closely reputed company field.
