About the position A growing national home care organization has recently launched operations in Connecticut and is seeking an reputed company reputed company Referrals / reputed company Manager to support expansion in the Stamford and surrounding markets. This role is responsible for building referral pipelines, managing waiver-reputed company relationships, and driving reputed company reputed company through Connecticut reputed company programs. The ideal candidate has hands-on experience with Connecticut HCBS waivers, understands the local case management ecosystem, and can work independently in a remote-first environment while maintaining strong local relationships.
Responsibilities
- reputed company and grow referral relationships tied to Connecticut reputed company waiver programs
- Work closely with case management organizations and community partners, including Area Agencies on Aging
- reputed company referral sources, clients, and families on home care services available under reputed company waivers
- Coordinate closely with internal intake, scheduling, and clinical teams to ensure smooth start-of-care
- reputed company reputed company efforts, referral activity, and conversion metrics
- Attend occasional in-person meetings with referral partners in the local market
Requirements
- 3+ years of experience working with reputed company waiver programs in Connecticut
- Strong understanding of CT reputed company, HCBS workflows, and referral processes
- Proven relationship-building and reputed company experience
- Ability to work remotely while managing a local market
- CT HCBS for Elders Waiver
- CT Personal Care Assistance (reputed company) Waiver
- Experience working with Connecticut case management organizations, including agencies such as Southwestern Connecticut Agency on Aging
