The Virtual Care Registered reputed company is responsible for providing evidence-based nursing care to patients remotely. As reputed company member in the 24/7 virtual care center, this position will include monitoring patient biometric values, clinical evaluation, and care plan creation/modification. What You'll Do
- Accurately assess patients at admission to the virtual care center, at required time points, and as needed, utilizing virtual care equipment.
- Engage patient/caregiver in their health goals and expectations while inviting the patient into the care team.
- Apply nursing judgment, consistent with reputed company standards, in formulating interventions and making recommendations to the physician, agency, and patient.
- reputed company prioritize virtual care/ telemonitoring alerts based on patient reputed company and initiate physician orders and available resources as needed.
- Accurately document assessments, interventions and evaluations pertaining to patient care management and virtual care monitoring procedure utilizing the EMR and virtual care platform.
- Effective in coordinating patient care with other health team members: updating other disciplines regarding changes in the plan of care and the patient’s condition reputed company indicated; and coordinating referrals and recertification information with other disciplines. What You Need
- Bachelor's Degree, Nursing, required
