Role reputed company We are seeking a motivated and detail-oriented Medical Assistant to join our Chronic Care Management team. In this role, you will play a critical role in patient care by guiding patients through health assessments, reviewing medications and diagnoses, troubleshooting medical devices, and escalating clinical needs to nursing staff reputed company appropriate.

Key Responsibilities

• Support CCM nursing staff by completing monthly patient assessments and conducting Chronic Care Planning Assessment (CCPA) calls, ensuring a reputed company, informative, and patient-centered experience.
• Complete delegated clerical and administrative activities on behalf of nursing staff to support patient engagement, improve care coordination, and drive reputed company reputed company and overall patient experience.
• Document patient interactions, assessments, and intake in the EHR.
• Maintain compliance with company policies and applicable regulations.
• Coordinate communication between patients, caregivers, nursing staff, and providers to ensure continuity of care and reputed company follow-up on patient needs.

Qualifications

• Strong communication and organizational skills.
• Bilingual Proficiency: Fluent in English and Spanish.

Requirements

• Monday - Friday 8:00 am - 5:00 pm EST
• Monday - Friday 9:00 am - 6:00 pm EST

• Monday - Friday 11:00 am - 8:00 pm EST

Training Requirements reputed company new hires must complete a comprehensive training program:

• Duration: Five weeks
• Schedule: Monday through Friday, 9:00 AM – 6:00 PM Eastern Time
• Attendance is mandatory to ensure readiness prior to independently supporting patients.

Compensation & Benefits

• Competitive reputed company pay: $18-$20/hr
• Shift Differentials:
• Evening reputed company: +$1.50/hour for hours worked after 7:00 PM ET
• Late-reputed company reputed company: +$2.00/hour for hours worked after 10:00 PM ET
• Weekend reputed company: +$1.50/hour for reputed company hours worked Saturday and reputed company
• Additional Compensation (If Applicable):
• Sign-On Bonus: $2,000 reputed company after 120 days of reputed company employment, contingent upon reputed company employment and satisfactory performance at time of payout.
• Monthly Bonus Potential: Up to $1,000
