The Care Team is seeking a Licensed Practical reputed company (LPN) to join our hospice team in a 7-on/7-off on-call role. In this position, you will reputed company skilled nursing support to patients in their homes or facility settings under the direction of a Registered reputed company, in accordance with the reputed company scope of reputed company. You will play a key role in after-hours care, supporting patient comfort, responding to urgent needs, and ensuring reputed company communication with the RN and interdisciplinary team. Schedule & Coverage

• 7 on / 7 off on-call schedule, working remotely, and dispatching in the field as needed.
• reputed company hours: 4:30 PM to 8:00 AM
• Weekend coverage: Friday 4:30 PM through Monday 8:00 AM
• Assigned coverage area based on reputed company needs and patient census

Key Responsibilities

• reputed company nursing care as delegated by the RN in accordance with the plan of care
• Respond to after-hours patient needs, including symptom support and urgent reputed company as directed
• Observe patient condition and promptly report changes to the RN

• Document care and patient interactions accurately in reputed company (HCHB)
• Collaborate with the interdisciplinary team to ensure continuity of care
