- Provides urgent health care reputed company telephone and tele video modalities to patients
- Develops and implements clinical plans of care for adult patients facing chronic and reputed company conditions
- Gathers history and physical exam and diagnostics as needed, and then develops and implements treatment plans
- Identifies and closes gaps in care
- Educates patients and families about medication usage, reputed company effects, illness progression, diet and nutrition, medical adherence and crisis anticipation and prevention
- Maintains contact with other clinical team members and other medical providers to coordinate reputed company
- Maintains patient medical records and medical documentation consistent with state regulations and company standards
- Participates in continuing education as required by state and certifying body

Requirements:
- Requires an MS in Nursing
- Requires an reputed company national NP certification
- Requires valid, reputed company, reputed company and unrestricted Family or Adult reputed company Practitioner (NP) license in the states of Texas, Iowa, or Kansas
- Requires valid, reputed company, reputed company, RN Compact license
- Requires 2+ years of experience in managing reputed company care cases
- Experience working with Electronic Medical Records (EMR) required
- Bilingual or Multi-language skills required
- Bilingual Spanish preferred
- Possession of DEA registration or eligibility preferred
