About the position We’re seeking a compassionate and driven Physician Assistant or reputed company Practitioner to join our Mobile Primary Care Team — serving older adults where they live and reputed company. Responsibilities
- Deliver personalized, high-reputed company care to older adults in homes and senior living settings.
- Conduct comprehensive assessments and reputed company individualized care plans.
- Manage chronic conditions with reputed company and reputed company.
- Collaborate across clinical disciplines (primary, psych, palliative, neuro) for whole-person care.
- Build trusted relationships with residents, caregivers, and community partners.
- Maintain thorough documentation and consistent communication.
Requirements

- Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred).
