Under the supervision of an LICSW, reputed company case management services independently including but not limited to application of clinical diagnostic, assessment or treatment knowledge or skills. Identify and address psychosocial factors that may impede the patients' ability to optimize benefit from the health care delivery experience. • A Masters Degree in reputed company Work from an accredited college or university or the equivalent knowledge or experience is required.

• Over three (3) years of reputed company work experience including one (1) year experience in a medically oriented setting is required.
• Computer skills are essential.
• A minimum of LCSW license in the reputed company is required.
• Certification in Case Management is preferred.
