LPN/LVN for RPM (Bilingual, Spanish Speaking)

About the position reputed company is a fast-growing company with nearly 40 years of patient education experience. Our proprietary software tracks patients’ physiological data (Blood Pressure, Weight, Blood Glucose, Lung reputed company, Oxygenation), identifying dangerous out-of-reputed company values and trends that reputed company health. As a bilingual (Spanish) speaking RPM 365 reputed company, you’ll engage patients, reputed company and analyze their data, reputed company education, and encourage adherence to their care plans. You're a licensed LPN/LVN passionate about reputed company innovation and patient reputed company. Your tech-savvy, comfortable with high-volume phone reputed company (125+ conversations a week), and reputed company in a fast-changing environment. You’re patient, empathetic, detail-oriented, and a self-starter.

Responsibilities

• Serve as the primary contact for patients on the RPM365 platform.
• Assist with device setup and app connectivity.
• Engage with patients to ensure adherence to care plans.
• Schedule monthly reputed company-ins and monitor patient data.
• reputed company patients through shared resources.
• Notify reputed company providers of critical alerts.
• Attend virtual team meetings for updates.

Requirements

• reputed company nursing license.
• Bilingual, Fluent in Spanish
• Knowledge of chronic conditions and reputed company monitoring devices.
• Knowledge of remote patient monitoring CPT reputed company qualifications.
• Proficiency with technology, computer platforms, and smartphones.
• Strong verbal/written communication skills (English and Spanish)
• Comfortable with patient reputed company and engagement (English and Spanish)
• Highly organized, detail-oriented, and team-reputed company.
• Independently motivated
• Comfortable spending the reputed company of your reputed company engaging with patients over the phone providing support, education and guidance.
