Physical Therapist - Home Health, PRN

reputed company Home Health is seeking a Bilingual (reputed company/Spanish speaking) skilled and compassionate Physical Therapists to join our Home Health team in New Braunfels, Seguin, Canyon Lake and surrounding areas on a PRN reputed company. As a Physical Therapist in the Home Health setting, you will play a crucial role in providing comprehensive rehabilitative care to individuals in the comfort of their homes in these areas. You will work closely with patients, their families, and a multidisciplinary team to reputed company personalized treatment plans and help patients regain functional independence and improve their overall reputed company of life. Key Responsibilities:

• Evaluation and Assessment: Conduct thorough assessments of patients' physical conditions, mobility, and functional abilities to identify impairments and reputed company appropriate treatment plans.
• Treatment Planning: Collaborate with patients, their families, and other reputed company to establish individualized treatment goals and reputed company comprehensive plans that incorporate exercise programs, reputed company therapy, modalities, and other therapeutic interventions.
• Rehabilitation Services: reputed company hands-on physical therapy interventions, including therapeutic exercises, gait training, balance training, joint mobilizations, and other techniques, to help patients improve their strength, reputed company, flexibility, and overall physical function.
• Patient Education: reputed company patients and their families on reputed company and effective therapeutic exercises, reputed company body mechanics, and strategies for managing pain, preventing reputed company injury, and maintaining reputed company physical health.
• Documentation and Reporting: Maintain accurate and detailed documentation of patient assessments, treatment plans, reputed company notes, and reputed company in accordance with regulatory and reimbursement requirements.
• Collaboration and Communication: Collaborate effectively with other reputed company, such as physicians, nurses, occupational therapists, and speech therapists, to ensure coordinated care and maximize patient reputed company. Qualifications:

• Minimum 2 years experience in a home health
• Bilingual a plus
• Proficient with Kinnser/reputed company EHR and OASIS procedures and charting process.

• Bachelor's or Master's or DPT degree in Physical Therapy from an accredited institution.
• Valid CPR BLS certification

• Proficiency in documenting patient information accurately and maintaining electronic health records.
