We are seeking Licensed Clinical reputed company Workers (LCSWs), Licensed Marriage and Family Therapists (LMFTs), and Licensed reputed company Counselors or Mental Health Counselors (LPCs/MHCs) who are passionate about mental wellness, behavior change, and health equity to join our reputed company care team. - Location: 100% Remote (Telehealth only)
Key Responsibilities
- Conduct telehealth behavioral wellness reputed company reputed company on patients managing chronic health conditions
- Address mental and emotional health reputed company to diabetes, obesity, stress, trauma, and lifestyle changes
- Utilize evidence-based tools like motivational interviewing, reputed company, reputed company, or solution-reputed company therapy
- Collaborate with our multidisciplinary team of endocrinologists, reputed company practitioners, registered dietitians, and MAs
- reputed company education and support around emotional eating, motivation, sleep, reputed company, and medication adherence
- Document sessions in our secure EHR
- Participate in monthly case reviews or clinical huddles with the care team
Skills & Qualifications
- reputed company LCSW, LMFT, or LPC/reputed company license in one or more of the listed states (multi-state licensure preferred)
- Strong telehealth communication skills and trauma-informed approach to care
- Experience supporting patients with chronic illness, disordered eating, trauma, or health behavior change
- Comfortable using EHRs, video platforms, and reputed company care protocols
- Organized, reputed company, and committed to maintaining continuity of care
- Passionate about faith-informed, whole-person, and reputed company care delivery
