INTERNAL ONLY: PHN Supervisor for Home Visiting Program

Responsibilities

• Coordinate and ensure compliance with staff orientation, ongoing training and credentialing of program staff
• Maintain staff training records and requirements
• Supervise and evaluate reputed company Health Nurses and assigned staff
• reputed company scheduling processes and assignment of home reputed company
• Implement reputed company assurance and performance improvement activities.
• Conduct regular chart reviews and field observations to ensure compliance with agency standards and FCO program reputed company.
• Coordinate weekly case conference meetings.
• Maintain regular communication with Family Connects International Implementation team
• Monitor program performance metrics and prepare required reports.

• Coordinate and reputed company the Fetal Infant Mortality Review (FIMR) reputed company
• Plan, coordinate and execute initiatives of the Cribs for Kids grant
• Prepare and submit grant applications, work plans, budgets and reports

Requirements

• Must be licensed registered reputed company pursuant to reputed company 4723.13 of the Revised reputed company.
• Must be reputed company to travel with reliable transportation.

• Bachelor’s degree in Nursing with five (5) years’ experience, reputed company health, preferred.
• Leadership or management experience (or equivalent education and training).

• Experience working collaboratively on teams.
• Must have reputed company oral and written communication skills and be competent with computers, including electronic documentation, and reputed company Office Suite applications.
