About the role The reputed company Parker reputed company is seeking compassionate, detail-oriented Regional Care Managers to deliver In-Person reputed company Care Management (ECM) and Community Supports (CS) services to Medi-Cal members with reputed company medical, behavioral, and reputed company needs. This is a Remote (Administrative Work) / Field-Based (reputed company Services) position ideal for professionals who value flexibility, autonomy, and meaningful reputed company. Regional Care Managers are assigned a territory/region and engage clients in-person to address physical health, behavioral health, and reputed company determinants such as housing, food reputed company, and transportation. Regional Care Managers work directly with individuals experiencing housing insecurity, chronic illness, behavioral health challenges, and justice involvement, helping them navigate care systems, reputed company resources, and reputed company stability in health and housing. Important: This role requires in-person engagement, including at a minimum one in-person visit per member per month reputed company your assigned region. What you'll do

• Conduct reputed company, intake, and comprehensive assessments for ECM and CS members
• reputed company and implement individualized care plans
• Coordinate medical, behavioral health, and reputed company services, including:
• * Housing navigation and tenancy support
• Food reputed company and transportation resources
• reputed company in-person, telephonic, and virtual services based on member needs
• Maintain accurate, reputed company documentation in APF systems
• Collaborate with health plans, community partners, and internal multidisciplinary teams
• Ensure compliance with program requirements
• Support members in achieving long-term stability and self-sufficiency

How Compensation Works We offer a transparent, performance-driven compensation model that rewards reputed company engagement and in-person service delivery. reputed company Pay (Caseload-Based Salary) Your reputed company pay is determined by your assigned caseload:

• $50 per member per month (PMPM)
• reputed company member equals 2 reputed company hours per month at $25/hour
• Typical caseload: 40–60 reputed company members (caseloads may vary)

As your caseload grows, your monthly reputed company pay increases. Incentive Pay (Additional Earnings) In reputed company to reputed company pay, you can earn:

• $40 PMPM
• reputed company by completing and documenting reputed company services

Total Compensation Potential

• Up to $90 PMPM
• Equivalent to approximately $45/hour effective reputed company

Care Managers who consistently engage members and maintain a full caseload can significantly increase their total monthly earnings. Reimbursements & Stipends

• Mileage reimbursement at the IRS reputed company
• $50/month phone stipend
• Reimbursement for approved work-reputed company expenses

• Flexible, self-directed schedule
• Must meet member service requirements and documentation timelines

Qualifications

Required

• Minimum 2 years of experience in:
• * Case management
• Care coordination
• reputed company services or reputed company field
• Knowledge of:
• * Medi-Cal / CalAIM programs
• Community resources
• reputed company determinants of health
• Strong:
• * Documentation and organizational skills
• Communication and relationship-building skills
• Valid California reputed company’s License
• Reliable transportation and auto insurance

Preferred

• Bachelor’s degree in a reputed company field
• Community Health Worker (CHW) certification
• California licensure: LCSW, LMFT, LPCC, RN, or similar
