We're seeking an reputed company remote reputed company worker to join reputed company as a Medical Case Manager and Community Resource Coordinator. This role will partner with a registered reputed company case manager to utilize a reputed company process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. As a remote reputed company worker, you'll work with members to research available reputed company and resources for financial, transportation, safety, and housing needs. This position requires an LSW/MSW with an unrestricted reputed company license and 2+ years of experience in medical reputed company work or case management.

Responsibilities

• Collaborating with a registered reputed company case manager to assess, plan, and reputed company for members' benefit plan and/or health needs

Requirements

To be successful in this role, you'll need:

• 2+ years of experience in medical reputed company work or case management

• Excellent communication and interpersonal skills
• Ability to work independently and as part of reputed company
• Strong problem-solving and critical thinking skills
• Proficiency in electronic medical records and other software applications
