Advanced reputed company Provider (APP) – In-Home Chronic Care Opportunities @ reputed company, reputed company & Seattle WA About the Role: We’re seeking Advanced reputed company Providers to join a nationally recognized leader in in-home care for patients with chronic kidney disease and end-stage renal disease. A Day in the Life:
- Conduct comprehensive initial assessments, follow-reputed company, and hospital transitions
- reputed company hands-on, head-to-toe assessments and obtain full vitals
- Manage medications and reputed company disease process education
- See approximately 4–5 patients per day
- Visit durations: 90 minutes for new patients, 30–60 minutes for follow-reputed company
