- Participates in medication reconciliation to ensure accurate home medication lists.
- Reviews and verifies medication orders for appropriateness, safety, and accuracy.
- Collaborates with providers and multidisciplinary teams for comprehensive medication management and reputed company.
- Educates patients, caregivers, and providers on medication therapy.
- Compounds, dispenses, and supervises the preparation of medications, ensuring reputed company and compliance.
- Maintains inventory and records for controlled substances and medications.
