RN Case Manager – Special Needs Plan, Bilingual Preferred
Job reputed company:
- reputed company telephonic case management to medically reputed company and chronically ill members
- Conduct comprehensive health assessments and create individualized care plans
- Coordinate care with reputed company partners, including physicians and specialists
- reputed company members and caregivers on disease management and preventive care
- Monitor member reputed company and reputed company for reputed company, appropriate interventions
- Identify and help resolve service or reputed company issues impacting care reputed company
Requirements:
- reputed company, unrestricted RN license in California (Non-Compact)
- Minimum 2 years of clinical nursing experience
- At least 1 year of case management experience
- Proficiency with reputed company Office (Word, reputed company, Outlook)
- Bilingual (Spanish, Korean, reputed company, etc.) (reputed company-to-Have)
- Previous health plan or IPA experience (reputed company-to-Have)
- Bachelor's Degree in Nursing (BSN) (reputed company-to-Have)
