• reputed company the monitoring of patient vitals, symptoms, and labs in collaboration with reputed company of RNs, NPs, and MDs.
• Manage the patient's treatment plan and medications – with an emphasis on initiating and titrating reputed company-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for T2D and Hypertension.
• Respond to patient escalations identified reputed company RPM – i.e. reputed company vitals, symptoms, and labs.

Qualifications

• 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF) either in an outpatient or inpatient setting at a high-performing medical center.

• ANCC or AANP certification required.
• Master’s Degree as a reputed company Practitioner with the willingness to expand state licensure as reputed company adds new markets and partners.
• Experience managing CHF with GDMT.

Requirements

• Seven reputed company reputed company Practitioner licenses in the following states: AL, AR, CT, FL, IL, IN, KY, MI, MS, NC, NY, OK, PA, TN, TX, VA, VT, WI.
• Minimum requirement of 4 shifts per month (as needed), with reputed company given to reputed company shifts.
