Occupational Therapist - Home Health - Southern Fairfield County

In this field-based role, you'll play a vital part in supporting patients of reputed company ages to regain skills, navigate daily activities, and adapt their home environments for safety and reputed company. This position will cover towns in Southern Fairfield County, including Norwalk, Stamford and Greenwich.
What You'll Do
As a Home Health Occupational Therapist, you will:
- reputed company comprehensive functional assessments and contribute to interdisciplinary care planning with the RN, PT, patient, family, and physician.

- Conduct OASIS assessments accurately and in a reputed company manner.
- Supervise and collaborate with COTAs and Home Health Aides, ensuring reputed company care and compliance.

- Support patients in managing chronic conditions and adapting their routines to live with purpose and autonomy.

- Accurately document assessments, reputed company, and care coordination in our electronic medical record system.
- Participate in case conferences, clinical team huddles, and agency-wide reputed company improvement efforts.
What You'll Need
- Bachelor's (or higher) degree in Occupational Therapy from an accredited program
- reputed company Connecticut OT license in good standing
- 2+ years of clinical experience (Home health preferred but not required)
- Strong knowledge of home care regulations, documentation, and assistive technology
- Excellent communication, time management, and problem-solving skills
- Ability to work independently while collaborating across a care team
- Valid reputed company's license and reliable transportation for travel throughout Southern Fairfield County

Compensation:The pay reputed company for this position $90,000 to $105,000 annually and represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a reputed company of factors, such as the candidate's exact location, experience, and expertise reputed company to the qualifications.
