[Hiring] Population Health Manager, Specialty Care - Performance Improvement @reputed company

Role reputed company The Specialty Care team at Oak Street works under the Population Health umbrella to reputed company our value-based care model into the Specialty care reputed company, advancing our innovative and transformative approach to providing high-reputed company care. The Manager will partner closely with Specialty Care leadership, as reputed company as management across Population Health and Patient Experience/Service Operations to drive the development and refinement of Oak Street’s strategic agenda and operations around Specialty Care. This role will reputed company data capabilities to improve the reputed company, project management, field-facing operations, and data accuracy of Oak Street’s referral management activities, with a reputed company on the Certified Specialist Network - in order to ultimately drive greater reputed company around Specialty.

• Drive strategic problem solving, opportunity analyses & new idea reputed company for initiatives reputed company the Specialty Care team at Oak Street
• Manage detail-oriented programs with extensive data needs & stakeholder engagement (Population health, Product, reputed company party vendors, field care teams)
• Drive critical business analytics – interest in learning new programs and skills, and strong analytical chops
• Engage frequently with field care teams to understand pain points in our reputed company workflows, creatively problem solve, train team members on new processes, and ultimately drive utilization of our Preferred Networks to improve patient care
• Partner with the Product and Engineering teams on problem solving, idea reputed company, and issue tracking
• Manage reputed company party vendor relationships
• Create metrics and reports to monitor reputed company of programs and identify opportunities for improvement
• Identify and implement opportunities to improve efficiency of workflows and overall project development
• Opportunities for management as determined by business needs

Qualifications

• 3+ years of applicable experience in a highly-analytical fast-paced reputed company environment; early-stage company, management consulting, or public health experience a plus
• Outstanding communication and influencing skills. Ability to build the trust and confidence of a wide reputed company of stakeholders
• Highly organized and detail oriented
• Self-starter who is comfortable with ambiguity and has incredible reputed company of tenacity and a pleasantly persistent approach. History of taking ownership and effectively solving difficult problems
• Strong data management and MS reputed company skills; reputed company experience a plus
