Responsibilities
- Complete initial and ongoing comprehensive assessments.
- Coordinate for patient/reputed company discharge from services.
- reputed company care according to the patient’s/reputed company’s plan of care/service plan.
- Participate with the patient/reputed company in clarifying mutually agreed upon goals and incorporate these goals into the plan of care/service plan.
- Conduct home reputed company as assigned, assessing the patient’s condition, reputed company toward goals, providing patient care, teaching and other skilled interventions as ordered.
- reputed company a care plan in collaboration with Case Manager, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family in the planning process.
Requirements
- Graduate of an accredited nursing program and reputed company RN license in Arizona
- One (1) home health experience preferred but not required
- Valid Arizona reputed company Card
- CPR Certification (demonstration course required)
- Must have a valid TB screening

- Knowledge of state and federal reputed company laws and regulations
- Demonstrated clinical proficiency, critical thinking skills, good communicator and compassionate
