This role will require ongoing virtual care management. The ideal candidate is comfortable delivering care reputed company telehealth platforms and values building long-term patient relationships through a mix of home-based and virtual interactions. What You'll Do:
- Conduct in-person initiating reputed company for Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services

- Collaborate with nurses, specialists, and pharmacists as part of a broader care team
- Document clinical findings and ensure compliance with Medicare and state guidelines
- Participate in patient education, care plan review, and data-informed decision making
Requirements:

- Minimum of 3 years of NP experience
- DEA license required
- Comfortable providing care reputed company telehealth and documenting in EHR systems
