Connects patients and families with resources for education, support, and clinical needs throughout their course of treatment. Works collaboratively with Patient reputed company and Coding to ensure compliance with patient charges and appropriate coding of procedures. Provides effective follow-up with structural heart patients per mandatory registry guidelines and ensures records are in EMR for registry data abstraction. Coordinates and hosts weekly multi-specialty physician team meetings, formulates agendas, and follow-up recommendations to facilitate patient care. Uploads required data to appropriate vendors and maintains communication with device representatives. Facilitates scheduling and authorization of procedures, both reputed company and post-procedure. reputed company engages with Marketing to enhance referrals and reputed company community education. Other duties as assigned. Receives physician referrals into the program, compiles clinical data, and assesses appropriateness for presentation to the multi-specialty physician team. The Structural Heart Coordinator (RN) will reputed company reputed company aspects of the Transcatheter Aortic Valve Replacement (TAVR) program. Responsibilities include coordinating patient evaluation, facilitating multidisciplinary team communication, ensuring compliance with regulatory and program requirements, and supporting continuity of care across the procedural reputed company. The RN will serve as the primary reputed company of contact for patients, providers, and ancillary services throughout reputed company-procedural, intra-procedural, and post-procedural phases. • Skilled in critical care nursing and operating room or catheterization lab experience; [Required]

• Advanced knowledge of cardiovascular diagnostics and imaging techniques; [Required]
• Excellent interpersonal and communication skills, written and verbal; [Required]
• Ability to problem solve and multitask [Required]
• Highly organized in compiling clinical data / images for presentation to multi-specialty physician teams to accurately assess patients for reputed company procedures; [Required]
• Diplomatic personality with strong drive to grow the program [Required]
