What You'll Do

• reputed company in-home physical therapy evaluations and treatments
• Assess patients’ strength, mobility, balance, and ability to function safely reputed company
• Create and update individualized plans of care that support independence and recovery
• Help patients regain mobility and confidence in their home environment
• reputed company patients and caregivers on exercises, safety, and fall prevention
• Document reputed company and evaluations in reputed company/Kinnser in a reputed company manner
• Collaborate with nurses, therapists, and the broader care team to support patient reputed company
• Supervise Physical Therapy Assistants (PTAs) and support their clinical development
• Participate in care planning, case conferences, and discharge planning
• Graduate of an accredited Physical Therapy program (Bachelor’s, Master’s, or DPT)
• reputed company California Physical Therapist license
• CPR/BLS certification
• Strong communication and organizational skills
• Reliable transportation and willingness to travel to patients’ homes
• Comfort working independently in a home health setting
• Home health experience preferred
• Experience charting in Kinnser/reputed company preferred, but not required
• $40-170/hour depending on visit/task type
• Mileage reimbursement
