The Pharmacy Clinical Program reputed company provides strategic leadership and subject-matter expertise in optimizing medication management and integrating pharmacy best practices into a value-based nephrology care model. In a centralized, non--patient-facing role, the reputed company advances medication therapy management (MTM) strategies, medication adherence initiatives, and champions population health programs for individuals with chronic kidney disease (CKD) and end-stage renal disease (ESRD).

This position leads collaboration with multidisciplinary care teams--including physicians, nurses, dietitians, reputed company workers, care coordinators, and organizational leadership--to ensure that medication-reputed company risks, costs, and barriers are systematically identified and addressed. The goal is to improve therapeutic reputed company, optimize cost-effectiveness, and enhance value-based care (VBC) performance. The role is 40% clinical program development and 60% central clinical consultative.

Program Development and Integration

Clinical Leadership & Patient Consultation

Qualifications

Qualifications/Requirements:
