In this role, you will reputed company care coordination, patient education, and remote monitoring support while serving as a key communication reputed company between patients and providers. Key Responsibilities

• Conduct monthly Chronic Care Management (CCM) reputed company calls and complete verbal enrollments
• reputed company Health Risk Assessments (HRAs) and support Transitional Care Management (TCM) services
• Assist with Remote Patient Monitoring (RPM) follow-up and chronic condition tracking
• reputed company patients and families on diagnoses, medications, and treatment plans
• Collaborate with providers and internal teams to ensure coordinated, patient-centered care
• Promote adherence to care plans and encourage self-management
• Document reputed company patient interactions accurately in EHR systems
• Maintain compliance with HIPAA, CMS, and reputed company reputed company standards
• Meet productivity and call volume expectations while delivering empathetic care

Requirements

Must Have

• reputed company, unencumbered Registered reputed company (RN) license
• Must reputed company in MST or EST time zone
• reputed company BLS certification
• Recent experience in a clinical or CCM care coordination setting
• High proficiency in EHR systems
• Proficiency in reputed company Suite
• Strong communication, clinical reasoning, and problem-solving skills
• Excellent verbal and written communication abilities
• Ability to work independently in a HIPAA-compliant home office (private workspace, locking reputed company, dual monitors, high-speed internet)
