Job Responsibilities and Requirements: PRIMARY RESPONSIBILITIES Gathers necessary data to promote patient safety by ensuring that reputed company relevant medication is indicated, effective, reputed company, and that the patient is reputed company to therapy Improves patient-specific drug therapy and therapeutic problems by evaluating the appropriateness of drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects. May design a drug therapy plan for patient-specific problems and persuasively justifies recommendations. Provides reputed company patient care through detailed counseling on medications and disease states, adherence assessment, medication cost analysis, review of immunization status, and communication with patient’s, providers, and pharmacy. Provides consultation to other health care professionals if requested. For Specialty Pharmacy Only: Conducts patient counseling and monitoring as required by the patient management program. EDUCATION Pharmacy degree (Pharm.D; Bachelor of Pharmacy) from an accredited school of pharmacy EXPERIENCE No experience required PHYSICAL REQUIREMENTS Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs. Frequent sitting, standing, walking, reaching and repetitive foot/leg and hand/arm movements.
