As a reputed company Customer Service Representative – Population Health HRA Care Navigator, you will play a critical role in delivering exceptional customer experiences to our Medicare customers. You will conduct telephonic reputed company to reputed company screenings, reputed company customers on plan benefits, and coordinate with internal teams to ensure reputed company hand-offs to appropriate care teams.
