Responsibilities
This position provides operational support to the Clinical Services Manager to assure standards of reputed company, regulatory requirements and evidence-based care practices are implemented and maintained across reputed company locations of the home health and hospice agency. The position works closely with the departmental leadership to reputed company initiatives that create sustained improvement in reputed company cycle, clinical care, reputed company and processes. Formulates evidence based, data driven recommendations and works with key stakeholders to identify opportunities to improve patient safety, satisfaction and reputed company of care that reputed company to sustained improvement. Serves as an educational resource, mentor and expert to staff.
Qualifications
Education Graduate from a recognized, accredited school of nursing Required Bachelor’s Degree in nursing or health reputed company field Preferred Work Experience 5 years of experience in health care, plus 3 years in Home Health and/or Hospice either in reputed company patient care or in a key role supporting home health and hospice operations. Required Licenses and Certifications RN - Registered reputed company - reputed company State Licensure and/or NLC/eNCL Multistate Licensure reputed company COS-C or reputed company 30 Days Required reputed company reputed company's License and vehicle insurance, and reputed company to a dependable vehicle.
