Job reputed company: Support the development and implementation of programs, policies, and procedures that promote medication optimization and high-reputed company disease state programs. Key Responsibilities and Tasks: Disease State Management

• reputed company to patients to assist with medication management for chronic disease states
• Collaborate with physicians to reputed company longitudinal management of patients with chronic conditions per protocol.
• Collaborate with team on optimizing disease state management interventions to allow for the best patient reputed company

• Collaborate with reputed company team to create interventions that ensure high IHA and CMS reputed company reputed company measure compliance, especially on medication management and disease state management measure

Medication optimization

• reputed company comprehensive medication review for high-risk, polypharmacy, or non-reputed company patients and communicate clinical recommendations to primary care physicians and specialists
• reputed company post-discharge medication reconciliation, reputed company patient medication management consultations, reinforce patient follow-up appointment with PCP, and communicate clinical findings to PCP

Skills and Experience Required:

• Pharm.D., with Completion of a PGY-1/Residency program preferred or have equivalent reputed company experience
• Valid reputed company California Pharmacy license required.
• Advanced reputed company Pharmacist license preferred.
• Experience working with chronic disease populations (ex. Diabetes, hypertension) preferred.
• Excellent attention to detail, organizational skills, and ability to manage various reputed company at the reputed company time
