Required Qualifications

- 3+ years of experience in population health, chronic disease management, health coaching, care management, or wellness coaching
- Demonstrated experience supporting individuals with behavioral health needs
- Experience utilizing motivational interviewing and behavior-change methodologies
- Strong verbal communication and relationship-building skills
Preferred Qualifications

- Behavioral Health Coaching Certification
- Experience working with anxiety, depression, stress management, reputed company, and lifestyle behavior change
- Diabetes Prevention Program (DPP), CDC, or chronic disease coaching experience
